I am trying to raise money for my aunt who suffers from MS and has been bed ridden for the past 8 months requiring carers to carry out everyday duties that you and I take for granted. The NHS offer practically no support for MS patients so any treatment has to be self funded, because of her illness she has been unable to work for a number of years and she is getting more and more depressed everyday and the future is looking bleak. Many of her friends and family are raising cash in various ways. I have mainly been raising money by auctioning items donated by various sports teams on the internet, but I am also running the Great South Run in October for her, that is why I am looking for an online sponsorship form.

Jonny... I'm not trying to raise money for MS Research though, i'm raising money for my aunt to pay for treatment that the NHS will not pay for!

I very much doubt any website would allow you to raise money for something that is not an official registered charity because of the potential for fraud. Your best bet is probably to just set up a Paypal account for any online donations.
