Well this is what I would do:
The blue line is the Piccadilly Line.
The Green & Red lines are new metro systems operated by BAA

Pity people changing from T5 to T4 there! But Heathrow Airport is already working on its own little transport syste for connecting flights I think.
If a hub was created, it might as well be done at T123, the biggest terminal with two tracks to the rest of the Picc. A T6 would be created north-west of the current T123, yes.

I have updated my idea with a piccy.
Blue is piccadilly. Yellow is HeX/Connect. Green is Crossrail (Could go to T 1,2,3 instead). Pink is the line towards Staines/Woking. Red is the Heathrow "Circular".
Note: All lines go in both directions.

Crossrail will use the same track as Heathrow Express, though, and will be taking over Heathrow Connect.
There is speculation that it will also take over the existing Heathrow Connect and run those trains further into London.
Anyway, those aren't being constructed towards T5.

As someone else has bumped this I guess I can put forward my idea as I watched mass confusion on the Piccadilly yet again on Sunday.
What I would do is to reverse the Heathrow 4 loop, meaning that trains would run H1234 or H1235. The lay over at T4 would mean that you would avoid the confusion of trains pulling in at 123 then another train leaving first, and with every train going to H123 first you would cater for the masses.

Keeping all tube stations where they are geographically (You may add stations or merge them), The best new tube map - with new lines, revised lines etc. gets brownie points.
